Thank you for registering for the Central Texas Youth Lacrosse Association's 2015 Referee training certification clinic. Please fill out and print a copy of an IRS W-9 form that can be found HERE. As required by federal law the Association is required to issue a 1099 for contractors that earn $600 or more in a calendar year. You will not get assigned a game until a W-9 form is completed. Also, please fill out this FORM so we can get the contact information for all the referees for our database. The Class will be held on January 10th from 8:30am to 3:00 pm at Regents School of Austin, 3230 Travis Country Circle, Austin, TX 78735.
