Thank you for registering for the Central Texas Youth Lacrosse League's 2013 Referee training certification clinic. Please fill out and print a copy of an IRS W-9 form that can be found HERE. As required by federal law the Central Texas Chapter of US Lacrosse, Inc is required to issue a 1099 for contractors that earn $600 or more in a calendar year. You will not get assigned a game until a W-9 form is completed. The Class will be held on January 20th from 9:00am to 4:00 pm at Austin HS in the Maroon Room. Be on time!
