I affirm that the following statements are true and accurate:

 Your contribution will be used in accordance with North Carolina law.  North Carolina requires us to use our best efforts to collect and report your name, mailing address, occupation, and the name of your employer if your contributions aggregate in excess of $50 per election.  You may contribute from your personal funds and may not receive reimbursement for your contribution from any person or entity.  The Committee will not accept contributions from individuals under the age of 18.  The Committee may not accept contributions from corporations, labor unions, business entities, professional associations or insurance companies.  Contributions from lobbyists are PROHIBITED.  Maximum contributions to the Committee shall not exceed $5000 per election.  Contributions are NOT tax deductible for federal income tax purposes.
