I affirm that the following statements are true and accurate:

Federal Elections Commission disclosure and agreement with the person contributing to HerreraPAC.

* The contributor is fully authorized to make this contribution.
* I am a United States citizen or a lawfully admitted permanent resident.
* This contribution is being made from a personal account and not by a foreign account and is not being made on behalf of another individual or entity.
* I am not a federal contractor nor do I currently have a contract with the federal government.
* I am at least 18 years of age and can legally make this contribution to HerreraPAC.

Contributions to HerreraPAC are not deductible as charitable contributions for federal income tax purposes. An individual may contribute a maximum donation of $5,000 but may contribute $10,000 if done from a joint account, which the donation will count as a contribution from two different individuals. Please include the name of the two individuals. To complete a donation to HerreraPAC, you are required to fill out the information marked with an asterisk “*.”

By contributing, you are agreeing to be put on a contact list for HerreraPAC and that the campaign may contact you via phone, email, or by postal services. You have the right to opt-out of receiving email, mail, or phone calls by sending an email to contact@herrerapac.com. If you choose to do so, please explain in the email body that you wish you opt-out and include your name, address, and phone number.
